چکیده

Strokes involving the insular cortex tend to be more severe. Preliminary evidence suggests that insular involvement is associated with poorer functional outcome after ischemic stroke regardless of infarct size, as well as with higher case fatality rates. However, these associations are controversial. The pathophysiological mechanisms explaining the apparent association of insular involvement with poor ischemic stroke outcome remain unknown. Differences in outcomes between left and right insular cortex strokes arise because of the laterality of autonomic representation in the brain, although there is disagreement whether the right or left insula is the one most associated with poor prognosis. We therefore examined the associations between insular involvement, its laterality, and outcome at 6 months after stroke in the large prospective data set provided by the IST-3 study (Third International Stroke Trial).

Letter by Alves et al Regarding Article, "Effect of Right Insular Involvement on Death and Functional Outcome After Acute Ischemic Stroke in the IST-3 Trial (Third International Stroke Trial)".

BACKGROUND AND PURPOSE In patients with acute ischemic stroke, whether involvement of the insular cortex influences outcome is controversial. Much of the apparent adverse outcome may relate to such strokes usually being severe. We examined the influence of right and left insular involvement on stroke outcomes among patients from the IST-3 study (Third International Stroke Trial) who had visible...

The Effect of Intravenous Magnesium Sulfate in Improvement of Acute Ischemic Stroke Induced Disability: A Randomized Double Blinded Clinical Trial

Background: Stroke is the third main cause of death and chronic disabilities in adults, which requires finding neuroprotective drugs to reduce its mortality and morbidity.  Objectives: To determine the efficacy of magnesium sulfate as an adjunctive neuroprotective agent in patients with stroke. Materials & Methods: This randomized double-blind clinical trial recruited 120 patients with acute ...

محلول "گلوکز- انسولین‌ـ پتاسیم" در سکته‌ مغزی ایسکمیک: کارآزمایی بالینی

Background: Hyperglycemia after acute stroke is a common finding that has been associated with an increased risk of death. For the last several years, it was believed that post-stroke hyperglycemia may worsen brain infarction in animal models. According to previous studies, the anti-inflammatory effect of insulin has a protective role on ischemic tissues. Glucose-insulin-potassium (GIK) infusio...

Effect of Lithium on Brain-derived Neurotrophic Factor (BDNF) Level in Patients with Ischemic Stroke: A Clinical Trial

Background and Objectives: Previous studies have indicated that lithium may increase the level of the brain-derived neurotrophic factor (BDNF), which in turn improves the recovery of patients with stroke. In this controlled trial we evaluated the effect of lithium on BDNF serum level in patients with ischemic stroke. Methods: In this randomized controlled...

The Effect of Priacetam in Aphasia due to Acute Brain Ischemic Stroke: Clinical trial

Background & Aims: Stroke incidence is 15 million people world wide annually and one third of these patients suffer from aphasia. Aphasia is often associated with significant disability in patients. Costs due to cerebrovascular diseases play an important role in national health expenditures. We aimed to assess the effect of piracetam in treatment of aphasia. Methods: Patients with acute ischemi...
